This modification to zen cart will create a form that will allow you to collect information from an applicant for a wholesale or distributor account.  This information is then sent to you in email format, so that you can set them up with group pricing in your admin area.  Also included is the ability to add the customer in your zen cart admin and send them the standard "Welcome" email.

This form has fields for the following:  Name, Address, City, State, Zip, Email, Phone number, Comments box and 1 extra text element you require (EG:  Federal Tax ID Number). 

Should you need more than these fields, please email me with the details so I can quote you for the additional coding that will be required.

PLEASE READ:

In order to install this mod on your website, I will need FTP access to your server and admin access to your zencart admin area. Once payment has been made, I will email you asking for that information and let you know the timeline on when the mod can be installed.

The charge for this item is for my professional help / time / knowledge in getting the mod installed and up and running on your website.

The code for this mod is freely available on the zen-cart.com website in the downloads area. If you just need the code to install yourself - please go to the zencart website and download it.
